Participation Agreement *

By submitting this form, I affirm that I have represented myself and my work truthfully and accurately. PCSCAC reserves the right to decline entries deemed inappropriate for this event. I allow photos that might be taken of my work for PCSCAC and event publicity purposes. Submission of work will automatically waive any claim for damage, loss or liability against PCSCAC and/or PCPAA or the Park City Community Church. Insurance is the artist’s individual responsibility. Space will be assigned by the event organizers. All registrations and payment must be received no later than January 15, 2020. Registration fees for the 2020 Winter Arts Showcase are $175. The cancellation deadline is January 15th and refunds will not be given after that date. If for any reason you are unable to attend the event, as a courtesy to other artists, please notify us immediately at 435-647-9747. If an emergency arises during the event and it is necessary for you to leave, you must notify an event official before dismantle.
